Beneficiary Designation Form – This form is required ONLY if you’re married and designating someone OTHER than your spouse as your primary beneficiary.
HARDSHIP WITHDRAWAL REQUEST If you’re an active employee and need to access your account, you may qualify for a hardship distribution for one of the following reasons: 1. To purchase a primary residence, 2. To prevent eviction or foreclosure, 3. For post secondary education expenses for you, spouse, or dependent, 4. For funeral expenses, 5. For repair to a primary residence due to natural disaster.
